Toronto Central Palliative Care Network The Toronto Central Palliative Care Network is a membership-based organization with a fee payment structure. The membership organizations have a defined hospice/palliative care mandate and are not for profit. Membership is system oriented, patient and family focused, quality driven, relationship based and strategically aligned. The directive of this network is to foster formal partnerships and on-going collaboration to establish a model of care that coordinates the many palliative care services available in Toronto. Toronto Central Palliative Care Network provides leadership and structure in the provision of palliative care services through the collaboration and cooperation of palliative care providers and organizations from a variety of disciplines and care settings.
